Why am I being asked to give feedback?
Your experience as a patient matters. Providers collect feedback from patients, colleagues, and co-workers to better understand their practice.
Your responses are anonymous and combined into a summary report. The provider reviews this report with a facilitator to identify areas for improvement.
How do I give feedback?
You will have one or more ways to complete the 360 survey. These are:
- A link or QR code to follow and complete the survey online
- A paper survey to complete and return on site
- A phone number to call and complete the survey over the phone.
The survey will take about 10 minutes to complete.

Is my feedback private?
Yes. The survey does not collect your name or personal health information—only your experience of care. Your responses are anonymous, combined with others, and shared in a report without any identifying details.
