Patient information on providing feedback

Feedback helps practitioners learn and improve. To support their ongoing professional development, you may be asked to give feedback on your doctor, nurse practitioner, midwife or dentist.

Why am I being asked to provide feedback?

As a patient, you can provide valuable feedback on your care experience. In a 360° review, your provider will seek feedback from co-workers, colleagues and patients.

All feedback is anonymous. It is grouped together and shared with the provider in a report. The providers will discuss the report with a trained facilitator, to learn how they may be able to improve their practice.

How do I give feedback?

If your practitioner asks for feedback, you will have one or more ways to complete the survey. These are:

  • A link or QR code to follow and complete the survey online
  • A paper survey to complete and return on site
  • A phone number to call and complete the survey over the phone.

The survey will take about 10 minutes to complete.

Is my feedback private?

The survey does not ask for your name or any personal health information. The survey only asks about the quality of the care you received today.

No one will know how you respond. All patient feedback will be grouped together in a report. The report does not include names and your feedback is completely confidential.
